REQUEST FOR PROPOSALS RFP UT-13-02TH OCCUPATIONAL HEALTH & EMPLOYEE HEALTH CLINIC SERVICES The Utah Transit Authority (UTA) is requesting proposals until 2:00pm MST on Tuesday, February 19, 2013 to provide Occupation Health and Employee Health Clinic Services. Services listed below for Occupation Health and Employee Health Clinic Services should be proposed as separate services, but UTA would also like to consider proposals combining Occupational Medial and Employee Health Clinic Services. Proposal for the following services for Utah Transit Authority may be provided in one or more of the categories listed below. Occupational Health Service 1. Physical exams 2. Occupational injury, illness and trauma treatment 3. Consultation on occupational medical issues 4. Drug and alcohol testing 5. Physical ability testing 6. Medical Review Officer Services Employee Health Clinic Services - Services can be proposed for the following: 1. On-site or off-site clinic services as listed below: 2. Co-op clinic services where UTA partners with another employer to share clinic services as listed below: a. Urgent care b. Primary health care c. Acute and episodic health care d. This project is being financed by UTA with possible financial assistance from the Federal Transit Administration (FTA) and also in the future, the Federal Railroad Administration (FRA) of the United States Department of Transportation. The contract awarded will be subject to the financial assistance contract between UTA and the U.S. Department of Transportation. All proposers will be required to sign, if applicable, the following Federal certifications: Buy America Certification, Affirmative Action and Disadvantaged Business Enterprise Statement, Certification Regarding Debarment and Suspension, Certification of Restrictions on Lobbying, and Certificate Regarding Cargo Preference. The individual or firm selected will be required to comply with all applicable Federal and State laws, regulations and certifications, and equal employment opportunity laws and regulations. The Utah Transit Authority, in accordance with Title VI of the Civil Rights Act of 1964, as amended, and 49 CFR Part 26 will afford Disadvantaged Business Enterprises (DBEs) full opportunity to respond and will not discriminate against any interested firm or person on the basis of race, color, sex, age, religion, or national origin in the review of qualifications or contract award. Issuance of this RFP does not commit UTA to award any contract, to pay any costs incurred in preparation of a proposal, or to procure or contract for services or supplies.
