NOTICE An emergency hazardous waste permit (#UT-034-2012) has been issued to the University of Utah, Salt Lake County, Utah. The permit authorizes the University of Utah to stabilize, prior to off-site shipment and disposal, 18 potentially unstable hazardous waste containers described in submissions to the Division of Solid and Hazardous Waste dated October 11, 2012 and November 26, 2012. This permit was effective on Tuesday November 6, 2012 and expired on Thursday November 8, 2012. REQUEST FOR QUALIFICATIONS PRE-QUALIFY GENERAL CONTRACTOR FOR THE 4TH STREET CLINIC EXPANSION AND REMODEL INTRODUCTION The 4th Street Clinic is soliciting qualifications from General Contractors to provide construction services for the 4th Street Clinic remodel and expansion. The project consists of a remodel of the existing clinic (9,810 square feet) and expansion into an adjacent space (7,060 square feet). The project will provide expansion of the waiting room and laboratory space, and co-location of healthcare team members for greater efficiency. Build out of a dental clinic in the existing space will provide acute and preventive oral health services to the clinic population. Installation of new substance abuse/behavioral health clinic space will expand outpatient substance abuse services. Big-D Construction has been selected as the Owner's Representative to protect 4th Street Clinic's interests in the design and construction phases of the project. Big-D will NOT be eligible to be the general contractor on the project but will remain involved as the Owner's Representative throughout the project's duration. HKS Architects is the designer on the project. TWO STAGE SELECTION This solicitation is the first stage of a two-stage competitive selection process. Stage I will involve the short-listing of Contractors based on the selection criteria outlined in the Request for Qualifications (RFQ). The RFQ will be made available through the Owners Representative on January 23, 2013. All interested firms shall request the RFQ via email. (tyson.kearney@big-d.com) Only the three to five Contractors who are selected to advance pursuant to this RFQ shall be allowed to submit bids on the Construction Documents for the Stage II of the selection process.
